The HSE has apologised to a Limerick family after a review found missed chances to save their baby girl during labour. The High Court heard the hospital admitted serious failings.

The mother of the baby, who was 16-weeks-old when he died, has settled a High Court action against the HSE over his death.

The boy, who cannot be named, exhibits developmental delay, is non-verbal and has been diagnosed with autism spectrum disorder.
